Every hospital is required to report RL 1 through 5 to the Ministry of Health. Here is a summary of what each form contains and how to prepare them from data already recorded — instead of recapitulating manually.
RL 1–5

FAQ
SIRS (Sistem Informasi Rumah Sakit / Hospital Information Reporting System) Online is the reporting hospitals submit to the Ministry of Health. Hospitals are required to submit Recapitulation Reports (Rekapitulasi Laporan / RL) 1 through 5 to the Ministry of Health as well as to the Provincial and District/City Health Offices. Reporting rules follow the latest SIRS revision (e.g. Revision 6.3).
Broadly: RL 1 (baseline data & facilities), RL 2 (staffing), RL 3 (services), RL 4 (morbidity/mortality, including RL 4B for outpatients), and RL 5 (monthly data such as visits). Each has detailed sub-forms.
The Adievia SIRS Online module provides an operator console for the RL forms with Excel + PDF export, pulling data from operations already recorded in the system — reducing manual recapitulation. Final submission is still done through the Ministry of Health's official SIRS Online platform.
